Team Medical in Southampton is recruiting a Care Coordinator to join our friendly team based at our Southampton Offices in Redbridge. Office hours are Monday to Friday 08:30 - 17:30, finishing at 17:00 on Fridays.

You will plan schedules a month ahead, match staff skills to client needs, and liaise with external organisations and families while maintaining capacity plans and providing written reports on incidents.

Qualificações

• Grounding in Health and Social Care, ideally hold a QCF Level 2 or 3.
• Friendly and caring approach with ability to relate to people from diverse backgrounds.
• Excellent organisational skills.

Responsabilidades

• Plan ahead schedules for the following month and fill unallocated shifts.
• Match staff skills to client needs.
• Liaise with external organisations, clients and families.
• Maintain a capacity plan to support growth.
• Provide written reports on complaints, accidents and incidents.

Main Duties:

• Ensuring schedules are planned ahead for the following month. Filling unallocated shifts, developing a template of shift patterns for the staff. Good IT knowledge is a advantage.
• Matching the skills of our staff with the needs of our clients.
• Liaising with external organisations, clients and their families, having empathy and understanding their needs.
• Excellent telephone manner and communication skills are a must.
• Managing the changes of the schedules and keeping staff and clients informed
• Devise and maintain a capacity plan to ensure consistent growth
• Providing written reports of all complaints, accidents and incidents in accordance with company policies.
• Understanding of full care/support worker compliance, ensuring not to work a non-compliant worker.
• To ensure daily that all shifts are allocated before leaving the office.

Skills and experience:

• You will have had a good grounding in Health and Social Crae, ideally hold an QCF (or equivalent ) Level 2 or 3
• You will have a friendly and caring approach and the ability to relate to people from a variety of backgrounds.
• Great organisational skills
• Full UK driving license
