Job summary

Abacare in Wales is seeking a dedicated Care Coordinator to support our clients in the community across Wrexham. You will coordinate care rotas, liaise with care workers and ensure high-quality, person-centered support.

The role offers training, progression and a benefits package, including holiday entitlement, maternity/paternity pay and discounts with major retailers. Salary £26,300 including on-call on a rota basis.

• You will be an efficient, organized and experienced Care Coordinator who is passionate about providing the best in community care, thrives on managing rotas to an impeccable standard and takes pride in building relationships with your care workers.
