Job summary

CCH Group is seeking a Care Coordinator in Darwen to organize and deliver high-quality home care. You will manage referrals, create rotas, and liaise with social workers and GPs, while monitoring service users’ conditions to ensure optimal care across our community.

The role involves weekday hours 9am–5pm, with on-call duties on a rota basis, and requires a driving licence. Salary £25,364 plus on-call payments, and a range of benefits are offered.

Experience & Organisation: You are an efficient, organized Care Coordinator passionate about community care. You excel in managing rotas to impeccable standards and pride yourself on building strong relationships with care workers.
