Position Overview: We are seeking a compassionate, experienced and organised Care Coordinator to join our dynamic team. In this role, you will coordinate carer rotas and facilitate communication between clients, families, multidisciplinary teams, and carers to ensure clients receive high-quality care.

Responsibilities
  • Coordinating schedules, preparing rotas, and communicating with staff, clients, and their families.
  • Ensuring care workers provide the quality of care that meets company standards.
  • Managing day-to-day care packages and working with team members to address service user and carer concerns effectively.
  • Engaging in business development to foster the company’s commercial growth.
  • Mentoring and supporting Care Workers.
  • Staying informed about community resources and services that benefit clients.
  • Processing new client referrals.
  • Monitoring care visits and logging in/out times.
  • Being part of the emergency On Call rota.
Experience and Qualifications
  • A minimum of 2 years’ proven experience as a Care Coordinator.
  • Must have a Level 2 Diploma in Health and Social Care.
  • Excellent planning and organisational skills, with administration experience.
