Job description

Are you an experienced care professional who enjoys supporting others, leading a team and making a genuine difference to people's lives?

This is an exciting opportunity for two motivated individuals to join a community care service as Care Coordinators, combining hands-on care with team leadership, organisation and service development.

You'll play a vital role in helping individuals maintain their independence, achieve their personal goals and enjoy the best possible quality of life in their own homes and communities. If you're ready for a role that offers greater responsibility, variety and the opportunity to shape how care is delivered, this could be the perfect next step.

The Role

As a Care Coordinator, you'll oversee care provision across one or two locations, ensuring individuals receive consistent, high-quality support while helping care staff perform at their best.

This is a varied position offering a balance of practical care, staff supervision and administrative responsibilities. You'll have the opportunity to build strong relationships with the people you support, coordinate an effective team and contribute to the ongoing development of the service.

Your responsibilities will include:
  • Planning and coordinating staff rotas to ensure effective service delivery.
  • Supervising, supporting and guiding care staff.
  • Completing assessments and developing individualised support plans.
  • Carrying out regular reviews to ensure support meets changing needs.
  • Providing hands-on care and support within individuals' homes and the wider community.
  • Maintaining accurate care records, reports and essential documentation.
  • Promoting independence, physical and mental wellbeing, and opportunities to participate in work and social activities.
  • Supporting service improvements and helping implement new ways of working.
  • Participating in a small amount of out-of-hours duty when required.

Current shifts operate between 7am and 7pm, with plans to expand towards a 24-hour, seven-day-a-week service throughout the year.

What You'll Need

We're looking for someone who combines a compassionate approach to care with the confidence, flexibility and organisational skills needed to coordinate a team.

Essential requirements:
  • A Level 3 qualification in Health and Social Care or an equivalent relevant qualification.
  • Previous experience in health or social care, including experience suitable for supervisory responsibilities.
  • A full manual UK driving licence.
  • A willingness to work towards a Level 5 qualification.
  • Excellent communication and interpersonal skills.
  • Strong organisational skills and the ability to manage competing priorities.
  • A positive, flexible and proactive approach.
  • A genuine commitment to person-centred care and promoting independence.
What Makes This Opportunity Different?

These are newly created positions, giving you the chance to become part of an evolving service and contribute to how care is delivered in the future.

You'll have the opportunity to take ownership of your role, develop your leadership abilities and make a tangible difference to both the people you support and the colleagues you work alongside.

In return, you can expect:
  • £15.45 per hour.
  • A permanent contract with 37.5 hours per week.
  • Opportunities to develop your leadership and management skills.
  • Support to progress towards a Level 5 qualification.
  • A varied role combining hands-on care, coordination and staff supervision.
  • The satisfaction of helping individuals live more independently and confidently.

If you're an experienced care professional looking for a fresh challenge, greater responsibility and the opportunity to make a real difference, we'd love to hear from you.
