Job summary

Radfield Home Care, based in Chippenham, is seeking a Care Coordinator to organise day-to-day care services and support our care professionals. You will coordinate schedules, ensure dependable, person-centred care, and act as a principal contact for carers, clients, families, and healthcare professionals.

The role involves on-call shifts, rota management for 15 carers, and close collaboration with the Branch Care Manager to maintain high standards of service.

Qualifications

  • A minimum of 1 year's practical care experience, including supervisory (essential).
  • rota planning or scheduling experience is desirable.
  • Excellent knowledge of the local area.
  • No NVQ in Health and Social Care required.
  • Strong organisational and problem-solving skills; good communication.

Responsibilities

  • Create and manage rotas for carers to support continuity and minimise travel time.
  • Allocate carers to customers based on skills, experience and location.
  • Respond to staffing or customer changes quickly and effectively.
  • Handle calls, emails and messages promptly and professionally; escalate concerns as needed.
  • Record and escalate concerns, complaints and compliments per procedures.
  • Work with colleagues and management to deliver high-quality care services.

The Role:

As a Care Coordinator, you'll play a key role in the day-to-day organisation of our care services. Working closely with the Branch Care Manager, you'll coordinate care schedules, support our care professionals, and ensure customers receive dependable, person-centred care.

You'll also be a main point of contact for carers, clients, family members and healthcare professionals, dealing with enquiries, resolving issues and helping maintain excellent standards across the service.

Working Hours:

This is a full-time role based on a 37.5 hour working week and includes participation in a shared on-call rota covering weekday evenings and alternate weekends - 5 staff members have access to the on call mobile

Monday to Friday - 7am / 8am to 4pm

Additional shifts and ad-hoc cover may also be available, providing opportunities to increase your earnings.

As part of the on-call team, you'll provide out-of-hours support, responding to emergencies, staff absences and general enquiries to ensure continuity of care at all times.

What you'll do

  • Record and elevate concerns, complaints and compliments in line with company procedures
  • Work closely with colleagues and management to deliver a high-quality care service
  • Carry out staff supervisions and observations, supporting development and maintaining standards
  • Review and update care plans to reflect customers' individual needs and preferences
  • Identify opportunities to support branch growth through community engagement and business development
  • Deliver outstanding customer service in every interaction
  • Participate in the branch's shared on-call rota
About You:
  • A minimum of 1 year's practical care experience, including supervisory (essential)
  • Previous rota planning or scheduling experience (desirable)
  • Excellent knowledge of the local area
  • You don't require an NVQ in Health and Social Care
  • Excellent organisational and problem-solving skills
  • Strong communication skills with a calm and professional approach
  • Confident using care management or rostering software (training provided)
  • Able to work independently while contributing positively as part of a team

  • Salary of up to £30,000 per year based on experience, with additional earnings for out-of-office hours care visits
