Radfield Home Care, based in Chippenham, is seeking a Care Coordinator to organise day-to-day care services and support our care professionals. You will coordinate schedules, ensure dependable, person-centred care, and act as a principal contact for carers, clients, families, and healthcare professionals.
The role involves on-call shifts, rota management for 15 carers, and close collaboration with the Branch Care Manager to maintain high standards of service.

As a Care Coordinator, you'll play a key role in the day-to-day organisation of our care services. Working closely with the Branch Care Manager, you'll coordinate care schedules, support our care professionals, and ensure customers receive dependable, person-centred care.
You'll also be a main point of contact for carers, clients, family members and healthcare professionals, dealing with enquiries, resolving issues and helping maintain excellent standards across the service.
This is a full-time role based on a 37.5 hour working week and includes participation in a shared on-call rota covering weekday evenings and alternate weekends - 5 staff members have access to the on call mobile
Monday to Friday - 7am / 8am to 4pm
Additional shifts and ad-hoc cover may also be available, providing opportunities to increase your earnings.
As part of the on-call team, you'll provide out-of-hours support, responding to emergencies, staff absences and general enquiries to ensure continuity of care at all times.
Additional hours may occasionally be required to meet the needs of the business.
