Job Description
Key Responsibilities
- Coordinating care packages and ensuring clients receive the right support.
- Managing staff rotas, scheduling visits and responding to changes in care needs.
- Supporting care staff and maintaining excellent communication with clients, families and colleagues.
- Ensuring care is delivered in line with company policies and relevant regulations.
- Maintaining accurate records and assisting with the smooth day-to-day running of the service.
- Providing cover for client calls as a last resort when alternative staffing arrangements are unavailable, ensuring continuity of care for our clients.
- Working flexibly to meet the needs of the business, including occasional changes to working hours.
- Responding promptly to any staffing or client emergencies that may arise.
About You
- Have experience in a care coordination, care management or similar role.
- Are passionate about providing outstanding care and support.
- Have excellent organisational and communication skills.
- Are flexible and willing to work around the needs of the business.
- Are proactive, dependable and able to work well under pressure.
- Have a positive attitude and a genuine commitment to helping others.
- Are confident working independently and as part of a team.
- Local to Bridgend with knowledge of the surrounding areas.
