Job summary

AMS Case Management is seeking two Support Workers for a client in Bury, Greater Manchester. You will provide daily support in the client's home and community, assist with planning, and help with activities, including evenings and weekends.

A full induction will cover the client’s therapy programme and risk awareness. Experience with care, a full driving licence, and the ability to work flexibly are essential.

Qualifications

• Administer prescribed medications and maintain medication records.
• Provide support to the client to organise days, activities and plans.
• Support wellbeing and a safe home and community environment.

Responsibilities

• Administer prescribed medication and maintain medication records.
• Support client to participate in activities inside and outside the home.
• Promote client wellbeing and maintain a safe environment.
• Carry out domestic tasks like laundry, ironing and cleaning.
• Provide daily welfare reports and liaise with the case manager.
• Attend meetings and training as requested.
