Job summary

Community Case Management is looking to recruit two support workers to assist a 26-year-old client in the Hemel Hempstead area. You will follow the care plan, enable participation in activities such as swimming and social events, and provide compassionate support to the client and family.

The role requires a reliable, friendly, team‑oriented approach and a clean UK driving licence for day shifts to drive the client’s automatic vehicle. DBS checks will be part of recruitment.

Qualifications

• Clean UK Driving licence required for day shifts to drive client’s automatic vehicle.
• Ability to work within a team.

Responsibilities

• You will provide a supportive, engaging approach to the client by following his care plan and helping him enjoy activities.
• You will be reliable, kind and able to build a trusting relationship with the client, family and team.
• Nights and days shifts are scheduled; two team members will cover 7 nights per fortnight and day hours per rota.
