Countess of Chester Hospital NHS Foundation Trust is seeking a Cancer Support Worker to join the Skin Cancer Team and help coordinate patient pathways from diagnosis through post-treatment. The role requires strong organisational skills, discretion and effective communication with clinicians, patients and families.

The post holder will provide administrative support, signpost patients to support services, and contribute to a compassionate, patient-centered pathway within an NHS foundation trust.

Qualifikationen

• GCSE English and Maths required.
• ECDL or equivalent experience.
• Level 3 Health or social care qualification (e.g. NVQ or similar).
• Evidence of continued role development.
• Basic counselling.

Aufgaben

• Provide administrative and coordination support to Patients, Families, Consultants, Cancer Nurse Specialists and other staff involved in the Cancer Pathway.
• Ensure the efficient channelling and close co-ordination of the patient pathway with robust communication between medical staff, GPs, patients, relatives and other disciplines within the Trust.
• Demonstrate excellent organisational skills, be flexible, exercise initiative and maintain high professional standards and confidentiality.
• Signpost patients and carers to relevant sources of support and information.
