Job summary

The Princess Alexandra Hospital NHS Trust is seeking a motivated Cancer MDT Tracker to support patient care by monitoring cancer pathways from referral through diagnosis to treatment, in line with national waiting time standards.

You will use a range of hospital IT systems to track progress, update records in real time, and work with MDT Coordinators, clinicians, and the wider Cancer Services team to expedite investigations and appointments, providing accurate, timely information for safe

Responsibilities

• Accurately locate patient information from a range of systems to support the recording of all events along a patient's journey, highlighting any delays and escalating using agreed process.
• Proactively track and monitor patients along their cancer diagnostic and treatment pathways, ensuring timely progression in line with national Cancer Waiting Time standards.
• Accurately update and maintain patient records across a range of hospital systems, identifying and escalating any delays or pathway issues.
• Liaise with clinical teams, departments, and other Trusts to obtain information, confirm diagnostic plans, and help expedite appointments or investigations where required.
• Support MDT Coordinators with preparation for weekly Multi-Disciplinary Team meetings by identifying patients requiring MDT discussion
• Review and action correspondence received via the generic NHS.net mailbox ensuring all cancer patients are correctly tracked on the database
• Maintain the highest standards of data quality and confidentiality at all times
• Participate in team meetings and contribute to service improvement initiatives.
