The Christie NHS Foundation Trust in Manchester is seeking an MDT Co-Ordinator/Patient Pathway Tracker to manage cancer patient pathways from referral to treatment. You will coordinate MDT meetings, prepare and circulate patient lists, and ensure accurate input onto the local CWP system, with timely breach reporting.
You will work with clinicians, secretaries and nurses, maintaining records, and supporting national datasets while meeting the NHS waiting time standards.
As an MDT Co-Ordinator/Patient Pathway Tracker you will be responsible for completing, but not limited to, tasks such as: Tracking target patients in a timely manner from referral to first treatment. Attending regular meetings and provide updates on the patient pathway. Accurately maintain a patient tracking list. Liaise with other trusts. Being responsible for the facilitation of Multi-disciplinary team meetings ensuring MDT meetings run effectively and are well structured. Accurately prepare and circulate lists of patients for discussion at each multi-disciplinary team meeting (MDT) using information provided by secretaries, lead clinicians and nurse specialists. To attend the multi-disciplinary team meetings, keeping comprehensive records of attendance and outcomes of the meeting, and distributing to relevant team members. To ensure timely and accurate input of the outcomes of the MDT meetings onto local patient administration system, CWP.
