Job summary

Greater Preston Primary Care Network is recruiting an Health Care Co-ordinator to support Early Diagnosis & Prevention of Cancer across the PCN. You will help practices improve screening uptake, ensure robust referral pathways, and maintain cancer data systems.

Role involves liaising with GPs, practice nurses, pharmacists, and social prescribers to reduce health inequalities, monitor patients with suspected cancer, and contribute to learning events with partners across the network.

Qualifications

• Excellent communication with diverse healthcare professionals
• Able to analyse data and extract meaningful trends
• Good understanding of cancer screening programmes and referral pathways
• Experience with NHS admin or similar healthcare environment would be advantageous

Responsibilities

• Coordinate and manage Early Detection & Prevention of Cancer across the PCN
• Support robust referral practices for suspected cancer and follow national guidelines
• Develop and maintain relationships with GP practices, pharmacists, social prescribers and admin staff
• Track screening uptake, engage hard-to-reach populations, and reduce health inequalities
• Monitor patients with suspected/confirmed cancer to ensure timely care within Cancer Waiting Time targets
• Provide admin support for project evaluation and feedback
• Organise peer-to-peer learning events and share resources from Cancer Alliance and partners
• Maintain cancer data sets and coding accuracy across practices
