Job summary

Agincare Group in the Bexley area is seeking an Outreach Support Worker to provide person-centred support to individuals with Learning Disabilities, Autism, Mental Health needs and complex requirements, helping them lead independent, meaningful lives in their communities.

You will visit people in their own homes and local areas across Bexley, Bromley and Newham, offering daily living support and helping with social activities, personal care, routines, meals and budgeting.

Qualifications

  • A minimum of 12 months on your right to work
  • Full UK Driving Licence

Responsibilities

  • Encouraging people to play active roles in their communities and to increase independence
  • Supporting activities of daily living, including social activities, personal care, meal planning, budgeting
  • Communicating using people’s preferred methods
  • Engaging to improve quality of life for the people we support
  • Specialising in positive behavioural support approaches when needed
  • Recording health changes and updates in management records
  • Team player, able to work in a team or independently

As an Outreach Support Worker, you will provide high-quality, person-centred support to individuals with Learning Disabilities, Autism, Mental Health needs, and complex support requirements, helping them achieve their personal goals, develop life skills, and maintain meaningful connections within their community.

Where you’ll be working:

Bexley, Bromley & Newham

You will provide outreach support to individuals within their own homes and local community.

Encouraging– people to play active roles in the communities in which they live, to encourage independence.

Supporting – Activities of daily Living including social activities, personal care, routines, planning and preparation of meals and budgeting.

Communicating - effectively communicate using people’s chosen methods

Engaging - promoting and motivating people we support to enhance their quality of life

Specialising - supporting people who may need positive behavioural support approaches

Recording - keeping accurate records &reporting changes in health or circumstances of service users to management

Team player– Working as part of a team, as well as being able to work using your own initiative.
