Job summary

Surrey and Borders Partnership NHS Foundation Trust is seeking a compassionate, proactive Senior Support Worker to join our specialist social care home supporting seven gentlemen aged 60–76 with severe learning disabilities, autism and complex needs. You will help them live meaningful lives in a person-centred environment.

You will work in the home and community, assisting with eating, personal care, medications, health checks and care plans, and may lead shifts during handover.

Responsibilities

• Assist individuals with eating and drinking.
• Support personal hygiene needs and all aspects of personal care, such as bathing/showering and continence support.
• Support physical care needs with manual handling using equipment.
• Involve and engage individuals in all aspects of their care.
• Ensure the dignity and respect of individuals at all times.
• Act as Shift Leader and delegate responsibilities at handover.
• Support people with their medication.
• Contribute to and record health checks, risk assessments and care plans.

• Assist individuals with eating and drinking.

• Support physical care needs which may be due to a range of acute or chronic difficulties and includes manual handling using equipment provided.
• Involve and engage individuals in all aspects of their care.
• Ensure that the dignity and respect of individuals are maintained at all times.
• Act as Shift Leader and delegate responsibilities to colleagues at handover in line with people’s needs and preferences, ensuring the shift planner is updated.
• Support people with their medication.
• Contribute to, and record, health checks, risk assessments and care plans, using physical records and/or digital tools (depending on registered home).
