Coventry and Warwickshire Partnership NHS Trust is seeking a Health Care Assistant Band 3 for the Development Centre Day Service in Longford, Coventry. This role offers full and part-time posts with a service operating five days a week, Monday to Friday.

The post focuses on person-centred and holistic care for adults with learning disabilities and complex health needs. You will need excellent communication, the ability to work autonomously and as part of a team, and prior experience in a care

Qualifications

• Experience of working in a care setting.
• Excellent communication skills and ability to work autonomously as well as in a team.
• Care certificate on commencement in post.

Responsabilités

• To support service users within a day service setting and develop therapeutic relationships.
• Support service user to participate in community facilities and activities at day service.
• Carry out programmes involving care, teaching and behavioural management in group situations and on a one-to-one basis to meet individual needs in a person-centred way.
• Introduce service users to new environments outside the home and support integration into small group activities.
• Participate in care planning by facilitating, evaluating and reviewing plans with family and service user and other professionals.
• Make observations of service users, report unusual events, and keep records of daily reports and observation charts.
• Develop skills by participating in specialised training as required.
