Job summary

Marlow Medical Group is seeking an autonomous advanced paramedic practitioner to assess and manage housebound residents with acute, chronic and complex needs. You will conduct home visits and ward rounds, deliver urgent assessments, and support decisions on hospital admission within a primary care network framework.

Role requires independent prescribing, HCPC registration, and experience in frailty and long-term conditions; leadership in improving housebound care and coordinating

Qualifications

• Degree or diploma in paramedic science
• Advanced clinical practice qualification or Master’s degree or equivalent advanced practice training
• Independent prescriber qualification
• Level 3 safeguarding children and adults

Responsibilities

• Provide autonomous assessment, diagnosis, treatment and management of housebound residents with acute, chronic and complex needs
• Undertake home and housebound ward-round consultations
• Deliver urgent and same-day assessments, including after falls
• Manage acute illness and clinical deterioration, and support decisions on hospital admission
• Assess and manage frailty, long-term conditions and multimorbidity
• Support end-of-life and palliative care
• Reduce avoidable admissions and A&E attendances through proactive care planning
• Support structured medication review alongside clinical pharmacists
• Provide clinical leadership across housebound services and lead improvement projects on frailty and housebound care
• Coordinate referrals, transitions between care settings and multidisciplinary care planning
• Identify and elevate safeguarding concerns
• Teach, mentor and supervise colleagues, including paramedic students
