Qualifications

• NMC registration as a nurse is required.
• At least 1 year of experience in a primary care setting.
• Experience of working within a nursing team.
• Experience of dealing with vulnerable patients.

Responsibilities

• Assess, diagnose, treat and manage patients via face-to-face, telephone and digital consultations.
• Triage and prioritise patients by clinical need and risk.
• Manage acute and chronic conditions within competence and protocols.
• Prescribe, review and de-prescribe medicines safely.
• Make timely referrals across primary, community, urgent, emergency and secondary care.
• Develop personalised care plans and coordinate care across organisational boundaries.
• Support population health improvement and audit activities.
