Job summary

Pembroke Medical Group in Wood Green is seeking a visiting clinical care practitioner to extend the community matron service for housebound patients. The role involves initial assessment, diagnosis, treatment, and review of undifferentiated problems, including independent prescribing and emergency care where required.

Work will cover care homes, patients in their own homes, and the surgery, operating autonomously within a three-practice primary care network.

Qualifications

• Independent nurse prescriber qualification is essential.
• Mentor or teaching qualification required.
• Recognised NP qualification at degree level or equivalent.
• Clinical supervision training and experience preferred.
• Minimum 5 years' post-registration experience.
• Recent primary and community nursing experience.
• Extended and supplementary nurse prescribing experience.
• Leadership in quality initiatives such as clinical benchmarking.

Responsibilities

• Provide first-point-of-contact care for undifferentiated problems, from history to management plan.
• Deliver care to patients at home, in care homes, and at the surgery.
• Instigate and interpret diagnostic tests; prescribe within independent prescriber scope.
• Prioritise health problems and initiate emergency care when needed.
• Keep accurate records in line with Caldicott Principles and GDPR.
• Collaborate with GPs, nurses, and wider team; liaise with social services and secondary care.
• Assess and manage patients in the home environment.
• Support health promotion and self-management for long-term conditions.
