South Reading and Shinfield Group Medical Practice is recruiting a Paramedic Practitioner to assess, diagnose and treat patients autonomously in the surgery or community settings. The role includes clinical telephone triage, home visits to housebound patients and care homes, and running urgent care clinics within the multidisciplinary team.

Responsabilidades

• Assess, plan, treat and refer patients autonomously in surgery or community.
• Diagnose and treat via face-to-face consultation and clinical telephone triage.
• Carry out home visits to housebound patients, care home residents and those with pre-existing conditions.
• Run urgent care clinics and assess, diagnose and treat minor illnesses with referral within the team.
• Make referrals to primary, secondary and social care following pathways and protocols.
• Initiate diagnostic investigations when appropriate.
• Supply, administer and prescribe medicines under policies and standards.
• Keep complete and accurate records of every patient contact per guidance.
• Develop care plans with Partners and contribute to continuity of care.
