Job summary

The Methodist Church in Britain is seeking a HCPC registered paramedic for a fixed-term, home-visit role within the new primary care home visiting team at North Hampshire Urgent Care. You will autonomously assess and manage unplanned urgent care needs in housebound patients, with remote clinical supervision and close collaboration with GP practices.

Requirements include at least two years operational clinical experience in primary, urgent or emergency care, HCPC registration, and ability to work

Qualifications

• HCPC registered paramedic is required.
• At least two years of operational clinical experience in primary, urgent, or emergency care.
• Evidence of autonomous and team working; CPD current and ability to evaluate own practice safety and effectiveness.

Responsibilities

• Autonomously visit patients at home to assess and diagnose acute and subacute urgent and primary care presentations.
• Provide urgent primary care treatment and discharge or refer within scope under remote clinical supervision.
• Make direct referrals to primary, secondary and social services per agreed pathways.
• Assess wider care needs holistically and escalate to colleagues as needed.
• Maintain timely, high-quality records of every patient contact and work within multidisciplinary teams.
• Build partnerships with GP practices using the service and communicate clearly with patients and families.
• Comply with HCPC standards, NHUC policies, and participate in clinical audit and quality improvement.
• Support clinical practice, coach new colleagues and take part in supervision and appraisal.
