Sutton Primary Care Networks seeks a qualified paramedic to join the Acute Home Visiting Service for housebound patients. The role focuses on continuity of care at home, including assessment, diagnosis, treatment and follow-up, and close collaboration with GP practices and multidisciplinary teams.

The post requires own vehicle for home visits and at least two years post-qualification experience. The role involves working across networks and delivering high-quality care in home settings.

Qualifikationen

• Minimum two years post-qualification experience.
• Experience or good understanding of urgent care and primary care settings.
• Experience in policy review and clinical audit is desirable.
• Familiarity with EMIS or similar clinical systems.

Aufgaben

• Assess, plan and deliver care to meet individual needs with clinical interventions and follow-up.
• Work autonomously as an accountable paramedic practitioner, assessing, diagnosing and treating patients holistically.
• Carry out home visits for GP practice or PCN in line with clinical guidelines.
• Collaborate with doctors and allied health professionals to manage acute and chronic conditions.
• Educate patients and promote self-care and informed choice.
• Help reduce unnecessary conveyance and hospital admissions; support continuity of care.
