Responsibilities

• Assess, diagnose, treat and manage patients via face-to-face, telephone and digital consultations.

• Make timely referrals across primary, community, urgent, emergency and secondary care.

Job description

This Advanced Nurse Practitioner post sits within Marlow Medical Group, a large primary care provider serving over 38,700 patients. The post-holder works as an autonomous clinician, assessing, diagnosing and treating patients through face-to-face, telephone, digital and home consultations. The role blends same-day and urgent care with long-term condition management, prevention, clinical leadership and education, working alongside GPs, pharmacists and paramedic practitioners. Candidates need NMC registration with advanced practice or non-medical prescribing qualifications, plus a year of primary care experience.

What the job involves

• Assess, diagnose, treat and manage patients through face-to-face, telephone and digital consultations, and clinically appropriate home visits
• Triage and prioritise patients by clinical need and risk
• Manage acute illness, minor illness, deterioration and same-day presentations within competence
• Diagnose and manage chronic conditions using drug and non-drug approaches
• Request, interpret and act on investigations within agreed pathways
• Prescribe, review and de-prescribe medicines safely within competence and protocols
• Make timely referrals across primary, community, urgent, emergency and secondary care
• Provide specialist advice, treatment planning and care coordination for patients with complex needs
• Recognise and manage clinical risk, safeguarding concerns and deteriorating patients
• Involve patients and carers in shared decision-making and communicate sensitive information with compassion
• Develop and review personalised care and support plans, including care across organisational boundaries
• Support screening, vaccination, health promotion and population health improvement, including audit and quality improvement
