Zusammenfassung

Maidenhead Primary Care Network is seeking a Nurse Practitioner to deliver autonomous assessment, diagnosis and treatment of minor and complex needs in a primary care setting in Maidenhead. You will prescribe in line with protocols and lead care across home visits and the Same Day Access Clinic.

Applicants should have current NMC registration and a minimum of four years post-registration experience in primary or urgent care. Visa sponsorship is not offered.

Qualifikationen

• Masters or accreditation for the Advanced role (MSN or equivalent accepted for non-Advanced nurse practitioners).
• Current NMC registration is required.
• Minimum of 4 years' post-registration experience, including substantial autonomous experience in primary care, urgent care or community settings.

Aufgaben

• Deliver high-standard patient care using advanced, autonomous clinical skills.
• Assess, diagnose and treat patients with minor and complex needs, initiating referrals and planning care.
• Perform safe triage by telephone or face-to-face.
• Undertake home visits for practice and PCN patients, including assessment and ongoing management.
• Undertake care home visits for registered patients, including care planning.
• Deliver sessions in the PCN Same Day Access Clinic, managing acute and urgent presentations.
• Prescribe medication in line with evidence-based practice and protocols.
• Provide supervision and education to nursing and HCA teams, including protocol development.
