Job summary

Trinity Court Surgery, a training practice led by five GP partners serving about 21,000 patients, is seeking an Advanced Nurse Practitioner to work within a multidisciplinary primary care team. The post centres on autonomous clinical work, diagnosing, managing and prescribing for patients of all ages, by face-to-face, telephone or video consultation.

Applicants need an advanced practice Master's-level qualification and independent prescribing, with registration with the NMC/HCPC.

Qualifications

• Advanced Nurse Practitioner qualification.
• Master's level qualification in Advanced Clinical Practice or equivalent.
• Independent prescribing qualification.
• Qualifications meeting NHS England Advanced Practice framework eligibility.

Responsibilities

• Autonomously diagnose, manage, prescribe for and discharge patients of all ages with illness or injury.
• Manage a clinical caseload face-to-face, by telephone and through digital triage pathways.
• Take histories, examine patients with undifferentiated conditions, and formulate diagnoses and treatment plans for acute and chronic illness.
• Prescribe independently within current legal and local frameworks.
• Order, interpret and act on pathology and radiology investigations.
• Refer to secondary care or community teams and arrange emergency admissions when required.
• Keep accurate electronic patient records on EMIS Web and use SNOMED CT coding.
• Support the practice in meeting QOF and local health contract targets.
• Take part in and lead clinical audits to maintain evidence-based local guidelines.
• Work with GPs, the wider team and district nurses to coordinate care pathways.
• Provide clinical supervision and support to practice nurses and junior staff.
• Maintain professional revalidation in line with NMC standards.
