Springfields Medical Centre is seeking an enthusiastic Advanced Nurse Practitioner to join our team for 4–6 sessions per week, with Monday mandatory. You will carry out autonomous 15‑minute face‑to‑face assessments, diagnose, treat and manage patients with acute and undifferentiated conditions, delivering high‑quality, patient‑centred care.

The successful applicant must be a qualified Independent Nurse Prescriber, with home visits as needed, appropriate referrals, and timely review of pathology

Qualificações

• Registered General Nurse with valid NMC registration.
• Non-Medical Independent Prescriber status.
• Experience in managing long-term conditions.
• Experience in minor illness, clinical decisions and telephone triage.
• Full driving licence.

Responsabilidades

• Deliver face-to-face clinical assessments in 15-minute appointments.
• Diagnose, treat, and manage patients with acute and undifferentiated conditions.
• Undertake home visits when required and make appropriate referrals.
• Review pathology results and hospital correspondence to ensure continuity of care.
• Work autonomously within the practice team and contribute to service development.
