Springfields Medical Centre in Warrington is seeking an enthusiastic Advanced Nurse Practitioner to join our supportive team for 4-6 sessions per week. The role involves autonomous face-to-face assessments in 15-minute slots, with home visits and referrals to community services as needed.

Applicants must be qualified Independent Nurse Prescribers and capable of making clinical decisions. Experience with SystmOne is advantageous; training can be provided where necessary.

Formación

• Qualified Advanced Nurse Practitioner with autonomous, patient-centered care delivery.
• Experienced in assessing, diagnosing, treating, and managing acute and undifferentiated conditions.
• Able to work autonomously and collaboratively within a multidisciplinary team.
• Experienced in independent prescribing and reviewing pathology results.

Responsabilidades

• Provide assessment, diagnosis, treatment and ongoing management of patients.
• Undertake face-to-face consultations and home visits as required.
• Prescribe medications within scope of practice and order/interprete investigations.
• Maintain accurate records and continuity of care with secondary and community services.
• Support clinical governance, safeguarding and professional development within the practice.
