Responsibilities

• Provide a first point of contact for patients with undifferentiated and undiagnosed health needs.
• Provide domiciliary visits.
• Using advanced assessment skills of history taking and clinical examination, make a diagnosis and initiate an appropriate plan of care, including referral to other health and social care professionals as necessary.
• Provide ongoing education and health promotion in relation to primary prevention/secondary prevention.
• Organise and manage own patient caseload.
• Request, interpret and evaluate diagnostic tests to assess patients' clinical problems or monitor progress.
• Manage patients with long term conditions, including the development, implementation and evaluation of individualised treatment plans.
• To provide a holistic clinical assessment drawing on knowledge about the wider determinants of health and ill health.
• Independently prescribe medication within own scope of practice and undertake medication reviews in accordance with current guidance.
• Participate in the development and implementation of Practice/PCN and wider system clinical pathways and guidelines where appropriate.
• Provide clinical care within own scope of practice and recognise, assess and appropriately refer patients presenting with mental health or safeguarding needs.
• Provide expert clinical advice and support within the practitioners area of competence.
• Establish links with Secondary Care providers to help deliver changes in care pathways and protocols.
• Work with nursing/medical teams in providing proactive management of patients with chronic diseases in accordance with local/national policy.
• Collating data and involvement in Chronic Disease Management for disease specific areas including Hypertension, Diabetes, Cardiovascular Disease, Asthma and COPD as part of the Quality and Outcomes Framework.
• Reviewing patients on other registers including Mental Health, Frailty and Epilepsy Registers and inputting data where appropriate as part of the Quality and Outcomes Framework.
• Undertaking audit of patients requiring review of further intervention.
