Job summary

The Cornerstone Practice is seeking a Permanent Part-time Advanced Nurse Practitioner to join our established multidisciplinary team. The post involves independent assessment, diagnosis and treatment of acute and undifferentiated illness, routine and same-day appointments, and independent prescribing.

You will conduct house visits, weekly care home ward rounds and manage complex patient needs. Applicants must hold Independent Prescribing (V300) and a Level 7 advanced clinical practice

Qualifications

• Independent Prescribing qualification (V300) is essential.
• Recognised Level 7 advanced clinical practice qualification.
• Evidence of relevant continuing professional development (CPD).

Responsibilities

• Independently assess, diagnose and treat patients with acute illness and undifferentiated symptoms.
• Undertake routine, urgent and same-day appointments, including minor illness consultations.
• Independently prescribe medication and manage treatment plans.
• Conduct home visits for housebound patients.
• Undertake weekly care home ward rounds and assess residents.
• Request, interpret and act on investigations, including pathology tests.
• Arrange referrals to secondary care and community services.
• Recognise red flags and manage clinical risk with safe autonomous decisions.
• Manage patients with multiple long-term conditions and complex needs.
• Medication reviews and optimisation within clinical competence.
• Provide safety-netting, follow-up advice and patient education.
• Maintain accurate clinical records using SystmOne.
