Job summary

Fenland Primary Care Network is recruiting an Advanced Clinical Practitioner/Paramedic to work across its four member practices, including home visits and surgery clinics. The post involves assessing, diagnosing and treating patients, with prescribing and referrals as part of the role.

Independent prescribing and primary care experience are required. You will work within multidisciplinary teams, using your own vehicle for visits and contributing to high-quality patient care in the community

Qualifications

• Independent prescribing qualification (V300)
• Current UK professional registration
• Demonstrable experience of primary care
• Experience of facilitating change to improve clinical practice or audit

Responsibilities

• Assess, diagnose and treat patients in surgery clinics and home visits.
• Provide autonomous, accountable care including timely interventions and resuscitation when needed.
• Prescribe and issue medications under policy and local pathways within competence.
• Make referrals to primary, community, hospital or social care teams as appropriate.
• Maintain accurate health records and participate in MDT meetings.
