Job summary

Middleway Surgery is seeking an Advanced Nurse Practitioner for a 32-hour per week contract, focusing on autonomous assessment and management of acute and chronic conditions. The post includes prescribing, chronic disease reviews, and referrals within a supportive GP team.

The role requires an ALNP credential or equivalent, an independent prescriber registration, and experience in chronic disease management and patient safety. Aimed at enhancing primary care delivery.

Qualifications

• Master’s degree via ALNP credentialing or an advanced practice degree.
• Minor illness qualification.
• Enhanced-level working per the relevant framework.

Responsibilities

• Assess, diagnose and manage acute conditions; start treatment plans.
• Prescribe as an independent or supplementary prescriber and review medicines.
• Deliver chronic disease reviews and run chronic disease clinics.
• Request, process and interpret pathology results and act on them.
• Support home visits and online triage and duty team.
• Participate in clinical emergencies and support the team.
• Ensure patient safety and coordinate referrals when needed.
