Job summary

Sciensus is seeking a Specialist Nurse to provide high-quality care in patients’ homes across East/West Sussex, helping to enable early discharge and reduce hospital admissions. You will work within a supportive MDT, visiting patients in their own homes and across a defined area.

The role is 37.5 hours per week over 5 days, with occasional weekends, and requires a self-motivated, autonomous nurse with a patient-centered approach.

Qualifications

• NMC-registered nurse (RGN) with valid registration (NMC).
• Experience in an acute care setting with the ability to make independent clinical decisions.
• Experience undertaking clinical assessments and administering sub-cutaneous injections.
• Experience caring for IV devices and administering IV therapies.
• Must hold a full UK driving licence.

Responsibilities

• Deliver high-quality nursing care to patients in their home environment.
• Safely administer a range of therapeutic interventions.
• Undertake clinical assessments and make independent clinical decisions.
• Maintain accurate clinical records.
• Identify, report and escalate risks and incidents.
• Support patient safety and safeguarding in collaboration with the MDT.
