Job summary

West Suffolk NHS Foundation Trust is seeking a Band 5 Community Nurse to join a dynamic team delivering care in the community across West Suffolk. You will work with a multidisciplinary team and provide autonomous nursing under supervision, handling assessments, treatment plans and complex case management.

Qualifications

• RN qualification (Part 1 of register) or equivalent.
• Current or pending NMC registration.
• Good communication skills.
• Able to communicate effectively.
• Able to provide high standards of care.
• Ability to use own initiative.

Responsibilities

• Triage assess plan and implement and evaluate all aspects of patient care.
• Develop care plans to ensure continuity of treatment.
• Work integrated within the neighbourhood team to ensure holistic care is provided.
• Communicate complex and sensitive information to patients and carers and liaise with GPs, Social Services, inpatient teams and other professionals.
• Plan and prioritise workload and ensure safe delegation of care plans.
• Maintain patient-held records and input data daily using System One.
