West Suffolk NHS Foundation Trust is seeking a Band 5 Community Nurse to join the neighbourhood team. The role focuses on triaging referrals, assessing patients in their homes, planning care, and delivering treatments such as specialist dressings in rural settings.

Independent daily route planning and autonomous working are key aspects. Requirements include current or pending NMC registration and mentorship qualification to be completed after starting.

Qualifications

• Good communication and interpersonal skills essential.
• Strong observational and reporting abilities required.
• Excellent time management and prioritisation skills needed.
• Clinical reasoning for safe patient care essential.
• Dexterity for using specialist equipment needed.
• Ability to safely manoeuvre patients with handling aids.

Responsabilités

• Triage, assess, plan, implement and evaluate patient care.
• Develop care plans to ensure continuity of treatment.
• Work within neighbourhood team to provide holistic care.
• Communicate complex information, gain consent and complete capacity assessments.
• Liaise with GPs, social services, inpatient teams and voluntary sector.
• Triage referrals, prioritise workload and allocate team tasks.
• Manage daily route and document patient records in SystmOne.
• Provide assessments and specialist dressings as trained.
• Request supplies through electronic ordering systems.
• Follow infection control policies and governance procedures.
• Undertake appraisals and support staff and students.
