Job summary
Sciensus is seeking a Specialist Nurse to provide patient-centered care in patients' homes across Glasgow and surrounding areas. The role supports early hospital discharge and prevents unnecessary admissions, working within a multidisciplinary team to deliver safe and compassionate care.
The position is 30 hours per week over 4 days (including occasional weekends) with autonomous, varied responsibilities and strong development opportunities.
Qualifications
- Registered Nurse with current NMC registration.
- Experience in acute care settings and making independent clinical decisions.
- Experience delivering home-based nursing care and IV therapies.
Responsibilities
- Deliver high quality nursing care in patients' home environment.
- Safely administer a range of therapeutic interventions.
- Undertake clinical assessments and make independent clinical decisions.
- Maintain accurate clinical records.
- Identify, report, and escalate risks and incidents.
- Support patient safety, wellbeing and safeguarding.
- Collaborate with a multidisciplinary team to ensure coordinated care.
