NHS Greater Glasgow and Clyde is seeking an experienced nurse to assess and manage care for patients with acute illness at home. The role involves coordinating with families, carers, community services and hospital teams to deliver safe, person-centred care.

You will lead a team, maintain clinical records, and participate in audits and professional development. A strong emphasis on evidence-based practice and flexible working arrangements is included.

Qualifications

• Part 1 NMC Registration, or 2nd Level Registration with General Nursing experience
• Evidence of CPD and NMC Revalidation
• Good IT skills including Microsoft Office, email and internet use
• Excellent verbal and written communication skills
• Ability to work independently and use initiative
• Effective leadership skills
• Strong problem-solving and decision-making skills
• Ability to work effectively as part of a multidisciplinary team
• Commitment to participating in mandatory training and professional development activities
• Ability to provide basic life support and respond to emergencies

Responsabilités

• Assess, plan, implement and evaluate care for patients experiencing acute illness at home
• Collaborate with patients, families, carers, community services and hospital teams
• Manage own caseload and maintain accurate clinical records
• Support patient independence and respond to emergencies
• Contribute to clinical audits, service improvement and research
• Provide clinical leadership and supervise students/juniors
• Engage in ongoing professional development and adhere to professional standards
