Job summary

Pharmaxo Pharmacy Services is seeking a dedicated NMC registered nurse to join our Homecare team in South Wales. The Homecare Nurse will provide direct patient care, administer medications, and support patient independence in various settings including clinics, residential accommodations, and patients' homes.

Applicants should have excellent communication skills, a compassionate approach, and a commitment to safe, patient-centred care. UK registration is required and CPD engagement is encouraged.

Qualifications

• Substantial post-registration clinical experience.
• Self-motivated and able to work under pressure in a fast-paced environment.
• Ability to work with a diverse range of stakeholders.

Responsibilities

• Perform patient observations and ensure patients are fit to proceed with treatment.
• Administer medications and treatments as prescribed.
• Monitor vital signs and report changes to the medical team.
• Provide compassionate support to patients and families.
• Educate patients and families to promote understanding and independence.
• Maintain accurate clinical records and ensure confidentiality.
• Collaborate with interdisciplinary teams to prioritise patient safety.
