Job summary

Leaders In Care Recruitment Ltd is seeking a Complex Care Case Manager to oversee community-based care packages across Merseyside. You'll manage your own caseload, build relationships with clients and families, and lead care teams delivering high-quality, personalised support in people's homes.

This full-time, permanent role offers a salary up to £35,000, with a bonus and on-call payments, Monday to Friday, and 25 days holiday plus bank holidays.

Qualifications

• Experience coordinating or managing complex care packages.
• Knowledge of neurological conditions, physical disabilities or long-term complex healthcare needs.
• Experience supervising and supporting care teams.
• Good understanding of CQC regulations and safeguarding.
• Strong organisational, communication and leadership skills.
• Full UK driving licence and access to your own vehicle.

Responsibilities

• Managing a caseload of community-based complex care packages.
• Completing care assessments, care plans and risk assessments.
• Coordinating and mobilising new care packages.
• Leading, supervising and developing community care teams.
• Building strong relationships with clients, families and healthcare professionals.
• Ensuring compliance with CQC standards and safeguarding requirements.
• Monitoring care quality and driving continuous improvement.
