Leaders in Care is seeking a Complex Care Case Manager for West Yorkshire. You will oversee community-based care packages, ensuring high-quality, compliant delivery and leading care teams to meet client needs across a multi-disciplinary network.

The role offers Monday–Friday working, a salary up to £35,000 with a bonus and on-call payments, plus 25 days leave and a Company pension. You will build strong relationships with clients and professionals to drive improvements.

Formación

• Experience coordinating or managing complex care packages.
• Knowledge of neurological conditions, physical disabilities or long-term complex healthcare needs.
• Experience supervising and supporting care teams; good understanding of CQC regulations and safeguarding.
• Strong organisational, communication and leadership skills.
• Full UK driving licence and access to own vehicle.

Responsabilidades

• Managing a caseload of community-based complex care packages
• Completing care assessments, care plans and risk assessments
• Coordinating and mobilising new care packages
• Leading, supervising and developing community care teams
• Building strong relationships with clients, families and healthcare professionals
• Ensuring compliance with CQC standards and safeguarding requirements
• Monitoring care quality and driving continuous improvement
