Job summary

East London NHS Foundation Trust is seeking a Community Mental Health Nurse to work autonomously within the Central Bedfordshire Older People’s Community Mental Health Team in Dunstable. You will assess, plan, implement and review care, coordinating with service users, carers and multidisciplinary colleagues to achieve person-centred outcomes.

Qualifications

• Registered Nurse with NMC registration and current practice.
• Experience in community mental health or CMHTs is desirable.
• Ability to work autonomously and collaboratively with MDTs.

Responsibilities

• Manage a defined caseload and deliver person-centred, evidence-based care.
• Conduct comprehensive mental health assessments and care planning.
• Coordinate day-to-day clinical activity and support team functioning.
• Administer and monitor medications per medicines management policies.
• Provide clinical leadership, supervision and support to junior staff.
• Participate in audit, quality improvement and service development.
