Birmingham Community Healthcare NHS Foundation Trust is seeking a Band 5 Community Staff Nurse to back fill the DN role. The post holder will work within an integrated Multidisciplinary Team under the supervision of the Clinical Team Manager, delivering high quality care to patients in health centres and promoting wellbeing and independence.

The role involves working closely with GPs, Tissue Viability and vascular clinics, and coordinating patient care as part of the IMT.

• Deliver high quality care to patients within health centres.
• Work with the MDT to promote health and independence for patients on the team caseload, coordinating with GPs, Tissue Viability and vascular clinics.
