Job summary

West Suffolk NHS Foundation Trust is seeking a Community Nurse to join the Newmarket Integrated Neighbourhood Team. You will provide nursing services in the community, assess patients during home visits and develop individual care plans within a multidisciplinary team.

The role involves collaboration with Virtual Ward and Early Intervention Team, autonomous practice with support, and opportunities for professional development within our integrated care pathways across West Suffolk.

Qualifications

• RN qualification (Part 1 of register) or equivalent.
• NMC registration required or NQ Nurse awaiting registration.
• Mentorship qualification is desirable for the post, but not essential at interview.

Responsibilities

• Assess, plan and deliver healthcare for patients in the community.
• Work within the integrated team to facilitate early discharge from hospital and coordinate with the Virtual Ward team.
• Collaborate with multi-disciplinary teams to provide seamless, integrated patient care.
• Ensure safe use and maintenance of equipment and supplies, within budget constraints.
• Provide mentorship to students and new staff as appropriate.
