Job summary

HCRG Care Group is seeking a Band 5 community nurse to join the Amesbury Health Centre team. The post involves assessing patients, planning and delivering care, escalating changes, and maintaining comprehensive records.

You will work within a multidisciplinary team, shape service development, and contribute to student education. Applicants must have NMC first-level registration, a full UK driving licence with access to a car for work, and be able to work with clinical autonomy while

Qualifications

• NMC first-level registration as a Registered Nurse.
• Full UK driving licence and access to a car for work.
• Ability to work both independently and within a multidisciplinary team.
• Strong communication and organisational skills; proficient IT and record-keeping.

Responsibilities

• Participate in assessment and care planning with the team.
• Implement and evaluate care and treatment per the care plan.
• Escalate deviations or changes in condition to the Team Manager or GP.
• Communicate with patients, carers and the MDT.
• Prioritise daily work according to clinical and patient needs.
• Keep accurate patient records on electronic and paper systems.
• Deliver evidence-based care and best practice.
