Job summary

Lincolnshire Community Health Service NHS Trust is recruiting Registered Nurses to join the Louth and Wolds District Nursing Team. The role is a full-time permanent post, requiring you to work autonomously and as part of a team, delivering care to patients in their homes, care homes and clinics.

You will assess care needs, plan, deliver and evaluate nursing care using evidence-based practice, with a robust induction and preceptorship programme supported by an experienced education team.

Qualifications

• Current NMC Registered Nurse in the UK.
• Evidence of ongoing professional development (CPD).
• Mentorship/preceptorship qualification and use in practice.

Responsibilities

• Travel to patients' homes and clinic settings as part of community nursing.
• Deliver and evaluate nursing care using evidence-based practice.
• Provide wound/continence care, palliative care and chronic disease management with MDT colleagues.
• Participate in induction and preceptorship programmes for new staff.
