Herefordshire and Worcestershire Health and Care NHS Trust is seeking a Community Nurse to join the Neighbourhood Team in Bromsgrove. You will deliver nursing care at patients' homes, supporting recovery and preventing admissions, with shift patterns from 07:30 to 18:30 and flexible working options.

• Registered nursing qualification with current NMC registration.
• Experience in community or district nursing preferred.
• Ability to work within a multidisciplinary team and in patients' homes.

Responsabilidades

• Autonomous practitioner delivering rapid and planned assessment and treatment in patients' homes.
• Maintain high standards of patient care, privacy and dignity in line with NMC Code.
• Manage caseload, support complex needs, and provide supervision and handover within the team.
