Job summary

Lincolnshire Community Health Services NHS Trust is seeking a permanent full-time Community Nurse in the Louth and Wolds District Nursing Team. You will assess needs in homes, care homes and clinics, plan and deliver nursing care, and work within a supportive multidisciplinary team.

The role requires current UK NMC adult nurse registration, and you will cover shifts from 8am to 6:30pm across seven days, with the need for own transport due to rural areas.

Qualifications

• Mentorship/supervisor/assessor qualification and its use in practice.
• Evidence of continuous professional development.
• Post-registration training in palliative, wound, elderly or diabetes care.

Responsibilities

• Assess patients in their homes, care homes and clinics.
• Plan, deliver, and evaluate nursing care using evidence-based practice.
• Deliver wound care, continence care, palliative care and medication administration.
• Coordinate patient care with the multidisciplinary team.
• Build therapeutic, person-centred relationships with patients.
• Work autonomously and as part of a team.
• Travel to patients' homes and clinic settings.
• Cover shifts 8am–6:30pm, seven days a week.
