Torbay and South Devon NHS Foundation Trust is seeking a Band 6 Intermediate Care Nurse to support patients in their homes and prevent hospital admissions. The post involves working within the Intermediate Care Team with strong links to Community Nursing.
You will provide holistic nursing assessments and care planning, with autonomous practice and prescribing where eligible. The role includes 37.5 hours per week, flexible working, and weekend rota coverage in Newton Abbot.
Qualifications
- Registered nurse with current UK registration.
- BSc Hons in Community Health Nursing (District) or equivalent or working towards.
- Mentorship qualification or experience preferred.
Responsabilités
- Provide initial and ongoing assessment of nursing needs in the community.
- Work as an autonomous practitioner planning and implementing nursing treatment.
- Advise and instruct carers and patients on health and wellbeing and self-care strategies.
- Carry out specialist nursing assessments including medication review, prescribing and palliative care.
