Job summary

Cornwall Partnership NHS Foundation Trust is seeking a compassionate Community Nurse (Band 5) to join the Launceston and Tamar Valley team. You will work 37.5 hours per week across Callington/Launceston with shift patterns including evenings and weekends, delivering patient-centered care in homes and community settings.

The role offers autonomous practice within a supportive multidisciplinary team, with strong emphasis on learning, development and flexible working arrangements.

Qualifications

• Registered Nurse (Adult) with current NMC registration.
• Evidence of recent professional development.
• Mentor qualification or willingness to work towards it.

Responsibilities

• Provide care for patients in their homes and community settings.
• Assess, plan, implement and evaluate care with a multidisciplinary team.
• Deliver high-quality, evidence-based nursing care across Callington and Launceston areas.
• Promote independence, wellbeing and self-management for patients and carers.
