Job summary

Cornwall Partnership NHS Foundation Trust is seeking a caring Community Nurse Band 5 for Bodmin and surrounding areas. Based at Bodmin Health Office, you will cover Bodmin, Par, Lostwithiel and nearby locations, collaborating with GP and social care colleagues to deliver high-quality, evidence-based community nursing care.

Qualifications

• Registered Nurse (Adult) with current NMC registration.
• Evidence of recent professional development and willingness to mentor students.
• Post-registration diploma or degree in nursing or working towards degree.

Responsibilities

• Assess, plan, implement and evaluate nursing care in patients' homes.
• Promote informed choice and support for patients and carers.
• Work autonomously within policies and procedures.
• Flexibly support locality teams based on service demand.
