Job summary

Torbay and South Devon NHS Foundation Trust is seeking an Urgent Care / Intermediate Care Clinical Nurse to work 22.5 hours per week for 12 months, covering maternity leave. The role spans across the community, including the Urgent Care Response Team and the Intermediate Care pathway, with the rehab unit at Jack Sears if required.

Based primarily at Kings Ash House, Paignton, you will travel to patients across Torbay and supervise non-registered staff, planning care to avoid hospital admission

Qualifications

• Qualified Registered Nurse with CPD ongoing.
• BSC (hons) in Community Health Nursing (District) or equivalent.
• Nurse prescribing or willingness to undertake training.
• Understanding of Health Care services and frailty/long-term condition management.
• Full driving licence and access to transport.

Responsibilities

• Undertake holistic nursing assessments and determine the most appropriate plan of intervention.
• Provide ongoing assessment of nursing needs in the community.
• Delegate tasks to non-registered team members within scope of practice.
• Work autonomously to plan and implement nursing treatment programmes.
• Advise and instruct carers and patients on health and well-being and self-care.
